PERSONAL HEALTH INFORMATION
Management of personal health received at our practice is treated with the strictest confidence. Confidentiality Agreements are in place with our staff. This information is required for our doctors to provide quality patient care. Please note you will be automatically be placed on the National Reminder System unless you specify otherwise. If a copy of your health information is required these are obtainable, however specialist letters, pathology and radiology can be obtained from them directly.
The ‘personal information’ we collect includes your name, date of birth, address/es, contact details, Medicare number, healthcare identifiers and health fund details. Medical information may include medical history and any care you may need. GP’s need information about your past and present health in order to provide you with high-quality care. Our practice follows the guidelines of the RACGP’S handbook for the management of health information in general practice, 5th edition (the handbook). The Handbook incorporates federal and state privacy legislation, and the Australian Privacy Principles, which requires that your personal information is kept private and secure.
DOCTOR’S POLICY RECEIVING & RETURNING ELECTRONIC COMMUNICATIONS INCLUDING TELEPHONE CALLS
Our practice values your privacy. At times, we may email you from our secure email system or practice software. You may also receive SMS messages if you have opted-in to receive them. Please note, to protect your personal information, we are unable to accept email communication from patients.
Telephone calls all URGENT matters will be triaged and if required will be put through to the doctor. For non urgent assistance you may leave your contact details and a short message with reception staff and your call will be returned. Conditions apply for Telehealth consults, at the discretion of the doctors, over 70’s, Chronic Health and vulnerable.
APPOINTMENTS
Please call 5596 1333 to arrange an appointment. EMERGENCIES will be given priority. WALK IN PATIENTS will try to be accommodated; how ever, booked appointments and emergencies will have priority. LONGER APPOINTMENTS are available on request. Please advise reception staff if required. HOME VISITS OR OUT OF HOURS consultations for patients can be organised after speaking with Doctor.
RECALL AND FOLLOW UP APPOINTMENTS
We may contact you about your results or for a follow up appointment via SMS, phone and/or letters.
It is important to note that when we contact you via SMS or letter, we are doing so via our practice software systems. This means that your doctor has determined that it is important for you to return to see them in a timely manner.
To ensure we can contact you about your results or appointments, please advise our reception teams that you would like to opt-in to receive SMS communications and don’t forget to let us know if any of your contact information has changed.
FEES POLICY
This practice is mixed billing. Bulk billing for all valid Medicare Card Holders. Private Fees will be required for those who DO NOT have a Medicare Card. Payment MUST be paid at the time of consultations. NO ACCOUNTS ARE GIVEN.
NOT COVERED BY MEDICARE
Medicare DO NOT cover the cost for services of Mirena Insertions, Commercial Driver’s licences, and or Insurance-Private or Pre-employment Medicals. Fees to be advised on confirmation of appointment with doctor.
YOUR RIGHTS
If you have a problem, we would like to hear about it. Please talk to the Practice Manager or Doctor. It is the practices policy to request these complaints in writing. If you feel you need to discuss this matter outside the practice there are several options available including the Medical Registration Board, AMA or Office of the Health Ombudsman.
